- Monthly membership includes:
- Annual labs: CBC, CMP, Lipids, hemoglobin A1c, TSH/ FREE T4/T3, urinalysis. Additional labs extra. Please keep in mind that we offer only the order for labs with the 3 month and 6 month memberships.
- Unlimited number of visits in office or via telehealth based on availability
- Recommendation of age-appropriate screenings and orders for preventative care. Cost of screenings not included.
- Ongoing care, prescriptions for medications and management of mild to moderate chronic conditions
- Referrals to specialists as needed
- Some minor procedures such as removal of stitches or staples and wound dressing changes.
- One time visit / Initial consultation fee $ 145 – can be applied towards any membership
- 3 month membership $ 135 a month – Includes the order but does not include the cost of annual labs
- 6 month membership $ 125 a month – Includes the order for labs at a discounted rate of $100
- 9 month membership $ 115 a month – Includes the order for and full cost of annual labs
- 12 month membership $ 105 a month – Includes the order for and full cost of annual labs
